FDA-approved on November 8, 2023 for adults with BMI ≥30, or BMI ≥27 with at least one weight-related comorbidity. On December 20, 2024, Zepbound received an additional FDA approval for treatment of moderate-to-severe obstructive sleep apnea in adults with obesity — the first medication ever approved for this indication.

Mechanism of action

Zepbound contains the same active ingredient as Mounjaro (tirzepatide), acting as a dual GIP and GLP-1 receptor agonist. Mechanisms relevant to weight management include central appetite suppression, slowed gastric emptying, and possibly direct effects on adipose tissue and energy expenditure under continued investigation.

Clinical efficacy data

SURMOUNT-1 (NEJM 2022): adults without diabetes lost a mean 20.9% of body weight on 15 mg over 72 weeks versus 3.1% on placebo. SURMOUNT-2: adults with type 2 diabetes and obesity lost up to 14.7% on 15 mg. SURMOUNT-4: continued treatment after a lead-in maintained weight loss; discontinuation led to substantial regain. SURMOUNT-OSA: reductions in apnea-hypopnea index supporting the December 2024 OSA approval.

FDA-approved indications

FDA-approved indications: (1) chronic weight management in adults with obesity or overweight with at least one weight-related condition; (2) moderate-to-severe obstructive sleep apnea in adults with obesity. Both indications are FDA-approved as adjuncts to a reduced-calorie diet and increased physical activity.

Dosing & titration

Identical titration to Mounjaro: 2.5 mg weekly for four weeks, then 5 mg, with stepwise increases of 2.5 mg every four weeks to maintenance at 5, 10, or 15 mg weekly. Pre-filled single-dose pens. Eli Lilly also offers single-dose vials at select doses through its self-pay LillyDirect program at reduced cost.

Safety profile & adverse events

Same safety profile as Mounjaro/tirzepatide. Most common: nausea, diarrhea, vomiting, constipation, abdominal pain, injection-site reactions, fatigue, hypersensitivity. Boxed warning for thyroid C-cell tumor risk; contraindicated with personal or family history of MTC or MEN 2. Monitor for pancreatitis, gallbladder events, severe hypoglycemia (when combined with insulin or insulin secretagogues), kidney injury from dehydration.

Cost & access in 2026

Zepbound list price is approximately $1,060 per month for pens. Lilly's self-pay vial program offers 2.5 mg and 5 mg vials at approximately $349–$549 per month for cash-paying patients. Commercial insurance coverage for weight management is inconsistent. Medicare does not cover Zepbound for weight loss; however, following the OSA approval, some patients with documented obstructive sleep apnea may obtain coverage through that specific FDA indication.
