News explainer · retatrutideRetatrutide in 2026: trial results, approval timeline and the research-peptide warning
Retatrutide is Eli Lilly's investigational once-weekly "triple agonist" (GIP, GLP-1 and glucagon receptors). As of October 14, 2026 it is not FDA-approved; Lilly plans to file for approval in the first quarter of 2027. In the phase 3 TRIUMPH-1 trial (topline May 21, 2026) adults without diabetes lost 28.3% of body weight on 12 mg at 80 weeks (efficacy estimand; 25.0% treatment-regimen) versus 2.2% on placebo. The FDA says retatrutide cannot legally be compounded, and "research peptide" retatrutide sold online is unapproved and has been linked to hospitalizations.
TRIUMPH phase 3 results so far
| Trial (topline date) | Population | Length | Weight change (efficacy estimand) | Stopped for side effects |
|---|---|---|---|---|
| TRIUMPH-1 (May 21, 2026) | 2,339 adults with obesity or overweight, no diabetes | 80 weeks | 4 mg −19.0%; 9 mg −25.9%; 12 mg −28.3%; placebo −2.2% | 4.1% / 6.9% / 11.3% vs 4.9% |
| TRIUMPH-2 (July 23, 2026) | 1,152 adults with type 2 diabetes | 80 weeks | 4 mg −12.7%; 9 mg −19.1%; 12 mg −20.8%; placebo −4.0% | 3.8%–11.6% vs 4.9% |
| TRIUMPH-3 (July 23, 2026) | 1,949 adults, BMI ≥35 with cardiovascular disease | 80 weeks | 9 mg −21.6%; 12 mg −22.6%; placebo −3.2% | 9.8% / 13.5% vs 4.8% |
| TRIUMPH-4 (December 11, 2025) | 445 adults with knee osteoarthritis | 68 weeks | 9 mg −26.4%; 12 mg −28.7%; placebo −2.1% | 12.2% / 18.2% vs 4.0% |
These are company-reported topline results; full peer-reviewed publications may differ. Lilly also reported dysesthesia (abnormal skin sensations): up to 12.5% on retatrutide versus 0.9% on placebo in TRIUMPH-1, and up to 20.9% versus 0.7% in TRIUMPH-4 (12 mg). For context, tirzepatide 15 mg produced 20.9% mean weight loss at 72 weeks in SURMOUNT-1 (treatment-regimen estimand) — different trials, populations and estimands, so the numbers are not directly comparable.
When could retatrutide be available?
Lilly says it plans to submit a Biologics License Application in the first quarter of 2027. A standard FDA review would put a decision after that; Lilly has not announced a launch date or price. Until approval, Lilly says retatrutide "is legally available only to participants in Lilly's clinical trials."
"Research peptide" retatrutide is not a shortcut
The FDA's GLP-1 concerns page (updated October 1, 2026) says "retatrutide and cagrilintide cannot be used in compounding under federal law." On August 24, 2026 the FDA sent warning letters to five online peptide sellers, including one listing "GLP-3 Reta (Retatrutide)," rejecting the "for research purposes only" label because the sites' own evidence showed the products were intended for people. Public Citizen reported in June 2026 at least 14 hospitalizations linked to unapproved retatrutide products. Products bought this way have no verified identity, dose or sterility.
What to use now
If you want the strongest approved option today, tirzepatide (Zepbound) is FDA-approved for chronic weight management, and compounded tirzepatide is available from 503A pharmacies under the FDA's limits. Lilly's oral Foundayo (orforglipron) was approved April 1, 2026 and starts at $149 a month on LillyDirect. Compare tirzepatide providers, tirzepatide vs semaglutide vs the GLP-1 pill, or the Foundayo approval.
Frequently asked questions
Is retatrutide FDA-approved?
No. As of October 14, 2026 retatrutide is investigational; Lilly plans to file for FDA approval in the first quarter of 2027.
How much weight do people lose on retatrutide?
In TRIUMPH-1, adults without diabetes lost 28.3% on 12 mg at 80 weeks (efficacy estimand) and 25.0% by the treatment-regimen estimand, versus about 2–4% on placebo, per Lilly's May 21, 2026 topline release.
Can I buy retatrutide peptide online?
Not legally for human use. The FDA says retatrutide cannot be compounded, and it sent warning letters on August 24, 2026 to sellers marketing it as a research peptide.
Is retatrutide stronger than tirzepatide?
Its trial numbers are higher, but no head-to-head phase 3 trial against tirzepatide had reported as of October 14, 2026, and the trials used different designs.
