Regulatory and legal record

An FDA inspection of Strive's Boerne, Texas pharmacy ended on September 8, 2023 with a Form 483; one observation describes a technician exposing "facial skin and hair while processing the drug product, Semaglutide/B12." The FDA closed out Strive's 2022 warning letter (639220) on May 13, 2026, stating "it appears that you have adequately addressed the violations." In Arizona, Sherwood News reported a February 7, 2024 consent agreement with a $1,000 fine; Strive described it as a documentation matter.

Eli Lilly sued Strive in Delaware over its compounded tirzepatide marketing; the court dismissed the case on October 8, 2025 for lack of personal jurisdiction. Strive's own antitrust suit against Lilly and Novo Nordisk was dismissed in August 2026, according to Reuters.

Glycine and B12 add-ins

Strive sells tirzepatide and semaglutide combined with glycine and vitamin B12. A February 2026 review in the Annals of Pharmacotherapy found "little justification for adding nutrients" to compounded GLP-1s, and the FDA said in April 2026 that semaglutide combined with B12 may still be "essentially a copy," and applied similar reasoning to tirzepatide/niacinamide in its September 2026 Empower letter. What the May 2026 close-out letter says

The FDA's close-out letter for warning letter 639220 is dated May 13, 2026 and was posted May 28, 2026. It is addressed to CEO Nathan Hill at Strive Pharmacy Texas LLC dba Strive Pharmacy and Vita Pharmacy, LLC dba Talon Pharmacy of Boerne. The address given is 3975 East Mesquite Street, Gilbert, AZ 85296. Matthew J. Lash, Acting Director of the FDA's Office of Compounding Quality and Compliance, signed it.

The FDA said it "has completed an evaluation of your firm's corrective actions" and that "it appears that you have adequately addressed the violations." It added that the letter "will not preclude any future regulatory action" if violations are found in a later inspection. The close-out applies only to the August 29, 2022 warning letter. It says nothing about the quality of current products, and it does not make compounded tirzepatide FDA-approved.

Is there a Strive 503B outsourcing facility now?

The FDA's registered outsourcing facility list, updated October 29, 2026, includes Strive Outsourcing LLC, Alachua, Florida. Its initial registration date is October 23, 2026, and it is marked "Not yet inspected." We have not confirmed whether it belongs to the same company as Strive Pharmacy in Gilbert. The Strive pharmacies covered on this page remain 503A.

A 503B registration would not, on its own, let a facility make tirzepatide from bulk ingredient. The FDA says tirzepatide is on neither the 503B bulks list nor the drug shortage list. On April 30, 2026 the FDA proposed keeping tirzepatide, semaglutide and liraglutide off the bulks list. Comments closed July 30, 2026, and no final decision had been published as of November 4, 2026. See what the bulks-list proposal means.

Strive's main tirzepatide product contains glycine and B12. In its September 18, 2026 warning letter to Empower, the FDA said tirzepatide/niacinamide and semaglutide/B12 products "appear to be essentially copies" of approved drugs. That letter did not name Strive, but it shows how the FDA now treats additive formulations. If your prescription is for tirzepatide/glycine/B12, ask your prescriber to state the clinical reason for the additive.

Questions to ask a program that fills through Strive

  • Is my prescription for tirzepatide alone or for tirzepatide/glycine/B12, and what is the concentration?
  • What clinical reason has the prescriber recorded for the additive formulation?
  • Which Strive location will fill it: Gilbert, Boerne or another site? Each has its own license and FDA history.
  • Does Strive hold a license in my state? Its site says 50 states, but its provider finder lists 42 states plus DC and Puerto Rico.
